Diabetes Prevention Program
Designed for patients who are pre-diabetic, our Diabetes Prevention Program helps you avoid a diabetes diagnosis through long-term nutrition changes, physical activity changes and support through stress management and group therapy sessions.
Eligibility requirements:
- Must be 18 years or older
- Overweight with a BMI of 25<
- No prior Type 1 or Type 2 diabetes diagnosis
- Must have pre-diabetic blood test results or previous diagnosis of gestational diabetes
Diabetes Education Program
Designed for patients with Type I or Type II diabetes, our Diabetes Education Program provides you the skills needed to manage diabetes while also improving your health outcomes through long-term lifestyle changes in nutrition and physical activity.
The program also includes group education sessions that teach you how to monitor and interpret blood glucose levels and how to detect and prevent acute and chronic complications of diabetes.
